  She disappeared, office closed, phones are down. Patients can't access their records, or request refills, or referrals, suboxone and methadone patients are out of luck. Patients under long term care are out of luck. the 1st time she packed up and moved her practice to another location without notifying anyone she just left a noted written in pen on the door to go here. No explanation just "oh I moved" then she dropped all of her patients with horizon without notice, then she goes M.I.A and there is no way to reach her.
